The Ayushman Vay Vandana card gives every Indian aged 70 or above free, cashless hospital treatment of up to ₹5 lakh a year at empanelled hospitals under Ayushman Bharat PM-JAY, whatever their income. As of October 2026 enrolment is open all year: a senior, or a family member helping them, can get the card in one sitting on the Ayushman app or at beneficiary.nha.gov.in with an Aadhaar-linked mobile number.

This guide is written for seniors and their children. It covers who qualifies, how the ₹5 lakh is shared, the step-by-step application, what to do when e-KYC or approval gets stuck, and how the card fits with private insurance. For the wider PM-JAY scheme (families, package list, hospital process), see our Ayushman Bharat PM-JAY guide.

What changed for seniors

The government expanded PM-JAY in October 2024 so that age alone makes a senior eligible. Before that, a senior qualified only if their household was in the deprivation-based list. Government replies in Parliament put the number of Vay Vandana cards at over 96 lakh by the end of 2025, and enrolment drives continued through 2026.

Eligibility in one line, and the fine print

Eligibility: you are an Indian resident and your age in Aadhaar is 70 or more. Nothing else: no income limit, no asset test, no exclusion for pensioners or taxpayers.

The fine print:

Your situation Can you get the card? Cover
Family already has a PM-JAY card Yes Separate top-up of up to ₹5 lakh a year just for you
Family not under PM-JAY Yes ₹5 lakh a year shared by family members aged 70+ (for example, husband and wife)
You have private health insurance Yes Card works alongside the policy
You are covered by ESI Yes Card works alongside ESI
You are in CGHS, ECHS, CAPF or a similar government scheme Only if you choose PM-JAY You must pick one scheme

Age is read from Aadhaar. If your Aadhaar shows a wrong date of birth, correct it at an Aadhaar Seva Kendra first; the Ayushman system will not accept a different age.

What the card pays for

The card works exactly like a PM-JAY card. It covers hospital admission for listed procedures at empanelled public and private hospitals, including surgery, ICU, medicines, diagnostics and implants within package rates, plus 3 days of pre-hospitalisation and 15 days of post-hospitalisation expenses. Pre-existing illnesses are covered from day one.

It does not cover routine OPD visits or medicines bought without admission, treatment at non-empanelled hospitals, or procedures outside the package list. Treatment is portable across states, so a senior living with a child in another city can use it there.

Worked example: a couple aged 72 and 70

Mohan (72) and Shanta (70) live in a family that is not on the PM-JAY list. Both get Vay Vandana cards, which share one ₹5 lakh family cover for the year (illustrative package costs):

Event Package cost Cover left
Mohan's cataract surgery (May) ₹15,000 ₹4,85,000
Shanta's hip replacement (September) ₹1,90,000 ₹2,95,000
Mohan's angioplasty (January) ₹1,30,000 ₹1,65,000

₹5,00,000 − ₹15,000 − ₹1,90,000 − ₹1,30,000 = ₹1,65,000 left, and the cover resets the next year.

Now suppose their son's family already has a PM-JAY card. Then Mohan and Shanta's ₹5 lakh is a top-up only for the seniors and does not reduce the son's family cover. Hospital package rates rise over time while ₹5 lakh stays fixed, so the cover buys less every year; our inflation calculator shows by how much.

How to apply for the Ayushman Vay Vandana card

Online, on the Ayushman app or beneficiary.nha.gov.in:

  1. Download the Ayushman app published by the National Health Authority from the official app store, or open beneficiary.nha.gov.in.
  2. Log in as Beneficiary with a mobile number and OTP. The mobile can be a child's number.
  3. Choose the 70+ / senior citizen enrolment option, select your state and enter the senior's Aadhaar.
  4. Complete Aadhaar e-KYC by OTP on the Aadhaar-linked mobile or by face authentication in the app.
  5. Capture or upload a recent photograph, and add the spouse if they are also 70+.
  6. Declare whether you are in CGHS, ECHS, CAPF or another government scheme, and confirm your choice.
  7. Submit. After approval the Vay Vandana card can be downloaded from the same login. Approval is often quick but can take a few days.

Offline: go to a Common Service Centre or the Ayushman Mitra desk at any empanelled hospital with the senior's Aadhaar and the Aadhaar-linked mobile. The senior may need to be present for fingerprint or face authentication.

Documents: Aadhaar card is the only essential document. Keep the Aadhaar-linked mobile handy for OTP.

Common problems and fixes

  • OTP not coming: the Aadhaar may be linked to an old number. Use face authentication in the app, or update the mobile at an Aadhaar centre.
  • Age shown below 70: Aadhaar date of birth is wrong or only partly recorded. Get it corrected in Aadhaar, then apply again.
  • Card "pending approval" for days: check status in the app; if it is stuck, visit the district PM-JAY office or call 14555.
  • Already in CGHS: you cannot use both. Compare your CGHS benefits (OPD, wider hospital choice) before switching.
  • Hospital asks for money for a covered procedure: do not pay; ask the Ayushman Mitra desk for pre-authorisation and complain if refused.

Vay Vandana card vs a private senior citizen policy

Feature Vay Vandana card Private senior citizen policy
Premium Free Often high at 70+
Pre-existing illness Covered from day one Waiting period, usually 1-3 years
Medical check-up to join No Often yes, or entry age limits
Hospitals Empanelled list only Insurer network plus reimbursement
Room category Package-based, often general ward As per policy
OPD Not covered Some plans cover it

For most seniors the card is a sensible base, whether or not they have a policy. Families who want private rooms or a wider hospital choice can keep or add a policy; our health insurance checklist helps compare. For income in retirement, the Senior Citizen Savings Scheme pays quarterly interest.

Where to complain

  1. At the hospital: the Ayushman Mitra desk and the hospital's grievance officer, in writing.
  2. National helpline 14555, or your state health agency's helpline; note the complaint number.
  3. Online grievance on pmjay.gov.in, which goes to district and state grievance committees.

PM-JAY complaints use this scheme route, not the Insurance Ombudsman.

Beware of fraud. The card is free and never needs "activation" or "renewal" payments. Do not share an OTP with callers claiming to be from Ayushman Bharat. Report fraud at cybercrime.gov.in or call 1930.

Key takeaways

  • Every Indian aged 70+ (as per Aadhaar) can get the Ayushman Vay Vandana card, with no income test, as of October 2026.
  • Cover is up to ₹5 lakh a year: a separate top-up if the family is in PM-JAY, otherwise shared among family members aged 70+.
  • Private insurance and ESI holders can also enrol; CGHS, ECHS and CAPF members must choose one scheme.
  • Apply free on the Ayushman app or beneficiary.nha.gov.in with Aadhaar e-KYC; CSCs and hospital Ayushman Mitra desks help offline.
  • Pre-existing illnesses are covered from day one, but OPD is not; complain via 14555 or pmjay.gov.in.

Frequently asked questions

Who can get the Ayushman Vay Vandana card?

Every Indian resident aged 70 or above, as per the age recorded in Aadhaar, regardless of income. There is no other condition; you only need to complete Aadhaar e-KYC.

How much cover does the Ayushman Vay Vandana card give?

Up to ₹5 lakh a year of cashless treatment at empanelled hospitals. If the family is already under PM-JAY the senior gets a separate ₹5 lakh top-up; otherwise the ₹5 lakh is shared among family members aged 70 and above, such as a husband and wife.

Can I get the Vay Vandana card if I have private health insurance?

Yes. Seniors with private health insurance or ESI cover can also take the card. Those covered by CGHS, ECHS, CAPF or another government health scheme must choose one: their existing scheme or PM-JAY.

How do I apply for the Ayushman Vay Vandana card online?

Download the Ayushman app or open beneficiary.nha.gov.in, log in with a mobile number and OTP, choose the 70+ enrolment option, complete Aadhaar e-KYC and upload a photo. The card can be downloaded once approved.

Is there a fee for the Ayushman Vay Vandana card?

No. Enrolment and the e-card are free. A Common Service Centre may charge a small fee for a printed PVC card, but nobody should charge for eligibility checks, e-KYC or approval.

Does the Vay Vandana card cover pre-existing diseases?

Yes. PM-JAY covers pre-existing conditions from day one with no waiting period, which is a big difference from most private senior citizen policies.
