Before taking a medical emergency loan, use money that costs nothing or very little: health insurance (cashless or reimbursement), government schemes like Ayushman Bharat PM-JAY if you are eligible, an employer advance, and your PF. For the gap that remains, a gold loan or a personal loan from an RBI-regulated bank or NBFC is usually safer and cheaper than high-cost app loans. Never let urgency push you into an "urgent medical loan online" from an unverified app.

This guide walks through the order to try these options, what each costs, and the red flags of fake medical loan offers. It is general information as of October 2026, not personal financial advice.

Step 1: Use health insurance first

If you or your family member has any health cover, whether a personal policy, an employer group policy or a family floater, start there before anything else.

Cashless claim

At a network hospital of your insurer, ask the hospital's insurance/TPA desk to send a pre-authorisation request right away. Under IRDAI's master circular on health insurance (May 2024), insurers are expected to decide on cashless authorisation requests within one hour, and on final discharge authorisation within three hours of the hospital's request. If the final authorisation is delayed beyond three hours, any extra amount charged by the hospital is to be borne by the insurer. Keep the policy number, ID card and patient's KYC ready.

Reimbursement claim

At a non-network hospital, you pay first and claim later. Collect every original bill, discharge summary, prescriptions and reports. This is where short-term borrowing may be needed, but only for the period until the claim is paid.

Know what will not be covered

Expect a gap even with insurance: room-rent limits, co-payment, deductibles, consumables and items excluded under the policy. Ask the hospital for an estimate and the insurer for the approved amount so you know the shortfall early. Our health insurance checklist explains these clauses. If a claim is rejected, complain to the insurer's grievance officer, then escalate through IRDAI's Bima Bharosa portal or the Insurance Ombudsman; see insurance claim rejected reasons.

Step 2: Check government schemes

Ayushman Bharat PM-JAY provides cashless treatment of up to ₹5 lakh per family per year for listed procedures at empanelled public and private hospitals. As of October 2026, it covers families identified through the SECC 2011 deprivation criteria (and state lists), families of ASHA and Anganwadi workers, and every Indian aged 70 or above regardless of income. Check eligibility on beneficiary.nha.gov.in or the Ayushman app; the hospital's Ayushman Mitra desk can also help. Many states run their own health schemes, sometimes merged with PM-JAY. Details are in our Ayushman Bharat PM-JAY guide.

Government hospitals also offer treatment at low or no cost, which can matter when no insurance applies.

Step 3: Interest-free or low-cost money

  • Employer advance or medical loan from employer: many employers give salary advances or interest-free medical loans to staff. Ask HR on day one.
  • PF partial withdrawal for illness: EPFO allows members to withdraw part of their EPF for the treatment of self, spouse, children or parents. Under the EPF Scheme, 2026, withdrawals are grouped into essential needs (which includes illness), housing and special circumstances. Reports on the new rules indicate a 12-month membership condition and that 25% of contributions must remain as a minimum balance, with the rest withdrawable subject to conditions. Apply online on the EPFO member portal or UMANG app with a UAN linked to Aadhaar and your bank account. Rules changed in 2025-26, so confirm the current conditions on epfindia.gov.in before applying.
  • Emergency fund and fixed deposits: breaking an FD early costs a small interest penalty, usually far less than a loan. A loan against the FD is another quick option.
  • Family support: if relatives help, write down the amount and repayment plan to avoid later disputes.

Step 4: Compare borrowing options

If a gap remains, compare these regulated options:

Hospital EMI or medical finance

Some hospitals tie up with banks or NBFCs to offer EMI on treatment bills, sometimes marketed as a medical expense plan or "zero-cost EMI". The lender is still a bank or NBFC, so ask for its name and the Key Fact Statement. "Zero-cost" often means the interest is offset by a discount you would otherwise get, or replaced by a processing fee; check the APR.

Personal loan

Unsecured, usually disbursed within a day or two by banks and faster for pre-approved customers. Rates depend heavily on your credit score and income. See our personal loan interest rates guide.

Gold loan

Often the fastest and cheapest option if you own gold jewellery, because it is secured. Under RBI rules, lenders can lend up to 85% of the gold's value for loans up to ₹2.5 lakh, and less for larger loans. The risk: if you cannot repay, the gold can be auctioned after notice. See the gold loan guide.

Credit card EMI

Paying the hospital by credit card and converting it to EMI is convenient, but the card issuer usually charges interest plus a processing fee, and GST applies on both. Paying only the minimum due on a large medical bill is very costly.

Worked example: ₹1 lakh gap over 12 months

Suppose insurance pays most of a ₹3 lakh bill, leaving ₹1 lakh you need to fund over one year. The rates and fees below are illustrative, not quotes:

Option (illustrative rate) Monthly EMI Interest Fees and GST Total cost
Gold loan at 10% ₹8,792 ₹5,499 ₹590 (₹500 fee + GST) ₹6,089
Personal loan at 14% ₹8,979 ₹7,745 ₹2,360 (2% fee + GST) ₹10,105
Credit card EMI at 16% ₹9,073 ₹8,877 ₹1,180 fee + ₹1,598 GST on interest ₹11,655
High-cost app loan at 36% ₹10,046 ₹20,555 ₹5,900 (5% fee + GST) ₹26,455

How the card row works: GST at 18% on ₹8,877 interest = ₹1,598; ₹8,877 + ₹1,180 + ₹1,598 = ₹11,655. The app loan costs more than four times the gold loan for the same ₹1 lakh. Check your own figures with the EMI calculator and compare APRs, not headline rates. A side-by-side of secured options is in personal loan vs gold loan vs loan against FD.

The RBI basics for any medical loan

Whether you borrow at a hospital desk, a branch or online, the same RBI (Digital Lending) Directions, 2025 protections apply to digital loans:

  • The lender must be an RBI-regulated bank or NBFC, named in the app or offer.
  • You must get a Key Fact Statement (KFS) with the APR, all fees and the repayment schedule before you sign. Fees not in the KFS cannot be charged later.
  • Money must be paid directly into your bank account (or, where you choose, to the hospital) by the lender, and EMIs go directly to the lender.
  • A cooling-off period, at least one day, lets you exit a digital loan by repaying principal and proportionate interest without a prepayment penalty. Useful if the insurance claim is approved after you borrow.

More detail is in instant loan apps: RBI rules.

Red flags of "urgent medical loan" apps and offers

Scammers target families in hospitals because they are stressed and in a hurry. Walk away if you see:

  • No named bank or NBFC, or a name that does not match RBI's list of digital lending apps.
  • Upfront payment demands for "processing", "insurance", "GST" or "file charges" before disbursal. Genuine lenders deduct fees from the loan and show them in the KFS.
  • Requests for access to contacts, photos or call logs. RBI rules bar this.
  • Payment to a personal UPI ID or someone else's bank account.
  • 7-14 day tenures with heavy "extension fees", or no KFS at all.
  • Unsolicited calls or WhatsApp messages offering a medical loan "approved in 5 minutes" after you posted about a patient online.
  • Fake crowdfunding or "hospital representatives" asking for OTPs or remote-access apps.

If you are cheated or harassed, report at cybercrime.gov.in or call 1930 immediately, and see our guide on fake loan app fraud and how to report it.

After the emergency

  • Repay the most expensive debt first, and use any reimbursement claim money to prepay the loan.
  • Check the loan account statement and your credit report once the loan closes.
  • Build or rebuild an emergency fund and review whether your health cover is enough; a super top-up can raise cover cheaply. See how much emergency fund you need.

Key takeaways

  • In a medical emergency, start with health insurance (cashless or reimbursement), then PM-JAY or state schemes if eligible.
  • Employer advances, PF partial withdrawal for illness and your own FDs usually cost less than any loan; confirm current EPFO conditions before applying.
  • For the remaining gap, a gold loan or personal loan from an RBI-regulated lender is usually cheaper than card EMI or high-cost app loans; compare APRs.
  • Hospital EMI and medical expense plans are loans too; ask for the lender's name and KFS.
  • Avoid apps that demand upfront fees, phone access or payments to personal accounts; report fraud on 1930 or cybercrime.gov.in.

Frequently asked questions

What is the fastest way to pay a hospital bill in an emergency?

If you have health insurance, ask the hospital's insurance or TPA desk to start a cashless claim immediately; under IRDAI's 2024 master circular, insurers are expected to decide on cashless requests within one hour. For the uncovered gap, an employer advance, a gold loan or a pre-approved personal loan from your own bank are usually the quickest regulated options.

Can I withdraw PF for medical treatment?

Yes. EPFO allows a partial withdrawal for the treatment of the member or family members, now grouped under essential needs in the EPF Scheme, 2026. Reports indicate a 12-month membership condition and that 25% of the balance must stay in the account. Check the current conditions on the EPFO member portal before applying.

Is a personal loan or gold loan better for medical expenses?

A gold loan from a bank or regulated NBFC is usually cheaper and quicker if you have gold, but you can lose the jewellery if you default. A personal loan needs no collateral but usually costs more. Compare the APR in each lender's Key Fact Statement.

Are urgent medical loan apps safe?

Only if the app lends on behalf of an RBI-regulated bank or NBFC, gives a Key Fact Statement with the APR before you sign, and pays directly into your bank account. Apps that ask for upfront fees, contact access or payments to personal UPI IDs are red flags.

Does Ayushman Bharat cover emergency treatment in private hospitals?

PM-JAY covers listed procedures at empanelled public and private hospitals, cashless up to ₹5 lakh per family per year, for eligible families and everyone aged 70 or above. Check eligibility on beneficiary.nha.gov.in and confirm the hospital is empanelled.
